Congrats on passing your exam! If you are a dues-paying member of SAM and have declared Actuarial Math as your major, please fill out this reimbursement form for reimbursement of your exam. Note that you will need your receipt and test result/confirmation letter. You do NOT need to wait for official results if you have confirmation of your UNOFFICIAL/PRELIMINARY passing.
